The following information is required for each participant:

Returning Participants

• Completed registration form
• Completed physical form (physical MUST by performed in calendar year 2010 and be completed by August 1st, 2010).  PHYSICALS MUST BE DOCUMENTED ON THE 2010 POP WARNER MEDICAL FORM – NO OTHER FORMS WILL BE ACCEPTED. (Please note: The 2010 Physical Fitness & Medical History form incorrectly states that it must be signed OR stamped by a medical professional. Current rules require that it is both SIGNED AND STAMPED.  2010 Physical Fitness & Medical History forms will not be accepted without both a signature and stamp from a medical professional!)
• 1” x 1” photo (head shot only)
• Three copies of 2008-2009 final grades for third graders and above (due in July)
• Volunteer bond check for $150 dated October 1, 2010 (one per family)
• Check for registration fees
• Original birth certificate (will only be needed in August)

New Participants

• Same requirements as returning participants
• Original & Copy of birth certificate

Registrations which are mailed in will only be accepted after March 10th. Any registration postmarked prior to March 10th will be returned.
